Observation

2)c) There were several cords around the licensed space indoors and outdoors that were capable of forming a loop around a child's neck and would be accessible to children. This included the cord to the electric recliner chair and several other cords plugged in near that chair; a power strip cord hanging behind open metal shelves in the dining area; the cord to the toaster, which was in a cabinet that would be accessible to children in the dining area; the cord to a cooking appliance on the counter in the kitchen/dining area; a cord to a white noise device in the play room infant area; cords from the lamps adjacent to the television in the play room, which were not well secured and could be pulled down; a cord to a shredder machine in the hallway; and an unsecured extension cord hanging down from the covered patio where children could access it. 2)d) There were several plastic bags and plastic items that could pose a risk of suffocation in the licensed space where they would be accessible to children. This included disposable plastic diaper mats and plastic bags in the unsecured bathroom drawers; plastic bags and plastic wrap in the kitchen drawers; and a couple plastic bags used for storage of various things in the playroom. 2)d) There was a plastic garbage bag in the outdoor play area that appears to have blown into the yard. It was in a location that would be accessible to children and needs to be removed. 3)a) There was a wooden bin next to the fence in the outdoor play area that had scrap wood in it. Some of the wood was splintered and had nails/screws in it. This would be accessible to children during outdoor play. 3)a) There was a paper shredder in the hallway where it would be accessible to children. 3)a) There was a barbecue cleaning tool on the table on the patio in the outdoor area that will be used for outdoor play. This tool had sharp points and edges that could pose a hazard of cuts/abrasions/punctures. 3)c) There are bifold doors in the bathroom and hallway that pose a pinching hazard. 3)c) The gliding rocker and footstool in the infant area have exposed moving parts that pose a hazard of pinching, sheering, and crushing if a child/infant put their hand or other body part into the gliding mechanism. 3)c) There was a can crusher mounted on one of the posts on the patio outside where it could pose a hazard of pinching, sheering, or crushing to a child. 5)a) There was an electrical outlet in the play room that had an accessory device plugged in with open outlets that were not tamper resistant. 5)a) There were two electrical outlets on the outside of the house that were not tamper resistant and would be accessible to children in care because the covers could not be secured and could be opened by a child. 5)d) and (f) In the playroom there were electrical cords from the lamps and from the door chimes plugged in to extension cords that were not being used for a brief, temporary purpose. The lamps and door chime were not plugged directly into a wall outlet or a surge protector as required. 5)e) There was a power strip on one of the metal shelves in the kitchen that would be accessible to children, particularly with the climbable surfaces nearby. 5)g) There was a night light and plug-in air freshener device plugged into the electrical outlet next to the bathroom sink, where children would have access as this is where they wash hands.
